ESOC 2022 | Endovascular therapy beyond large vessel occlusion… how can perfusion imaging help?

Pierre Seners, MD, PhD, Rothschild Foundation Hospital, Paris, France, discusses the potential benefits of perfusion imaging to extend thrombectomy eligibility. Endovascular treatment (EVT) is currently limited to patients with a small infarct core on baseline imaging. However, accumulating evidence is suggesting that the mismatch concept may play an important role in determining who may derive benefit from EVT. For example, recent studies in patients with a large core volume have shown significant benefit of EVT over medical management in patients with mismatch. The safety and efficacy of EVT is also not proven in patients with medium vessel occlusion. Using perfusion imaging to identify patients with at-risk tissue that could be saved with reperfusion may improve outcomes in these populations. This interview took place at European Stroke Organisation Conference 2022 in Lyon, France.
