Pavel Klein, MD, Mid-Atlantic Epilepsy and Sleep Center, Bethesda, MD, shares recommendations regarding optimized cenobamate treatment for epilepsy patients. Cenobamate can be a promising therapeutic choice for adults with epilepsy with focal onset seizures, with studies showing remarkable efficacy. Cenobamate should be used early, for instance, on patients who have not responded to two antiseizure medications. Clinicians should be aware of side effects, including the most usual ones such as somnolence, dizziness, and fatigue, as well as those related to sodium channel blockage. Drug-drug interactions should also be considered as they can lead to early pronounced side effects.
